3d scan and print a part for me

I have a plastic plate that I need copied in black.
The plate is about 5" x 4" (see pic for comparison), and the top surface (beige in this case) needs to be likewise-smooth on top (I don't mind sanding).
The web of support in the back (grey in these pics) is not necessary to be exactly accurate.
The rounded square, its height and smoothness, are what's required.
I'm thinking resin, but you tell me.
I'm looking to get 2 of these plates made in BLACK; basically copy the example.
To reply, be sure to tell me how long it'll take from the time I drop the part off with you, how much it'll cost out the door, and any other info you think would be handy.
